Anterior Cruciate Ligament (Acl) Injury

Overview

Anterior Cruciate Ligament (ACL) is one of the four major ligaments present in the human knee. A slightest of the injury to this ACL can be devastating for the patient. This injury is more often seen in athletes. Women are more prone to it due to many underlying factors. Even studies claim that women possess 8 times more risk of ACL injury than men. They face 25% more chances of a second time tearing of the ligament even after reconstruction surgery.

Causes

ACL injury is more common when athlete rapidly decelerates, followed by sudden change in the direction of movement. It may also happen to normal person when the knee accidentally gets twisted rapidly. Football or soccer players, basketball or tennis players, or volleyball players fall into risk zones much more than the rest. The National Institute of Health funded study is in the process to identify the unique movement patterns that are limited to women athletes which causes ACL injuries. To put it to the point we may say that ACL may occur during sudden dislocation, hyperextension and torsion of the knees. For women we can classify the causes into three main types:-

    * Environment- Any sports which includes running or jumping can be a possible threat.
    * Anatomical causes- There are certain major anatomical differences between male and female. The hip portion of female is wider, leading to straight legged stoppage and running. This increases the pressure of the knee muscles and risks knee injury.
    * Hormonal causes- Estrogen is a hormone which may put women in the possible dangers of the injury.

Signs & Symptoms

    * The patient suffering from ACL injury may initially hear a sudden “popping” sound followed by slight or severe pain.
    * One may also feel wobbly in the knee area.
    * Swelling of knees and bluish colour below the knee portion may also be seen in certain cases.

Prevention

Treatments
Conservative Management Modern Medicine

Non Surgical Treatment is also called “Conservative Management” treatment. Those patients are recommended this type of treatment whose knee cartilage is undamaged. If the injury is acute or you have a high activity level, surgery may be necessary; this may be either knee arthroscopy or "open" surgical reconstruction. Age has an effect on treatment - younger patients are more likely to have problems without surgery.

Physical Therapy Naturopathy

Initial treatment of an ACL injury includes splinting, ice to the area, elevation of the joint (above the level of the heart), nonsteroidal anti-inflammatory drugs (NSAIDs) and limited physical activity till the swelling reduces, motion is normal, and the pain is gone. Often the patient is recommended physical therapy to help regain joint and leg strength. They are also asked to use knee brace to recover quickly. While on treatment, the patient must avoid physical exercise as far as possible.

Surgery Modern Medicine

Surgery is required in more severe cases. Two types of surgeries are done namely allograft and autograft.
    * Allograft: is basically a cadaveric tissue taken from the tissue bank. There are chances when the graft gets rejected by the body.
    * Autograft is patient’s own tissues, mostly taken from hamstring tendons or the middle third of the patella tendon. Though, each of the two methods have their own set of advantages and disadvantages.
